This is a formal agreement between you, hereinafter known as DOCTOR, and Petty, Michel and Associates, P.O .Box 170882 (1031 E. Chateau Place), Whitefish Bay, Wisconsin, 53217, hereinafter known as PM&A.Whereas PM&A is a management and marketing group and whereas the DOCTOR wishes to engage the services of PM&A, it is hereby agreed as follows:
A. SERVICES INCLUDE:

Privacy. Complete
confidentiality will be observed relating to all matters of the DOCTOR and of the DOCTOR’s practice.

MMS Support and Coaching Program - $350 per month. ($250 if MMS CD or Binders purchased.)
B.
SERVICES BEGIN UPON FIRST PAYMENT.

Payment.
After your initial payment, future payments will be billed automatically to your credit card on day of the month you purchased the MMS Coaching Program, unless that date falls
after the 28th of the month, in which case your billing date will
be on the 28th of each month.
C. DOCTOR AGREES TO:

Monthly Statistics and Report.
Provide PM&A with the clinic’s major monthly statistics and marketing statistics (
Marketing Scorecard), as well as a marketing report (
Marketing Review and Game Plan) and to report these monthly statistics to our office no later than the 5th working day of each
month.

Client History and Information.
Provide PM&A with past clinic statistics and general confidential information.

Modifications.
You agree that PM&A may modify this
Agreement from time to time as needed.

Rights. You understand and agree that all contents and materials
contained in the MMS Coaching Program are protected by the various copyright laws
of the United States.
D. CONTRACT BREAKAGE:

There is no contract breakage fee.

Either
party may terminate this agreement at any time with email, fax, or
postal notification within 10 days prior to your billing date but no
later than 3 days prior to your billing date. There are no refunds for monthly fees once paid.
