Glad to be working with you -- and your team. It’s going to be fun, informative, revealing, and it may even take you outside of your “box” so that you will see your practice from a whole new perspective.
And most important, it will be effective. So, let’s go.
I
would first like you to fill out the
MMS Support and Coaching
Registration
Form. This link will take you to a document
that you can download. Please print it, fill it out completely, and fax it
back. But before you do that...
WAIT! You will be entering into the "MMS Members Section" and to do this, you will need a "user name" and a password. The default user name and password is:
User: mmsmember
Pass: reporting
Use this for now, though you may receive an email changing this in the near future. OK. Let's continue:
I will also need some information about the past performance of your practice. This link will take you to another document that requests
your practice statistics. It is called the “Practice Scorecard: Historical.”
Please fill it out and fax it back as soon as possible. This will now allow us
to put together a practice profile analysis from which our monthly evaluations
will be based upon.
Get these to us as soon as
possible. Then, I will send you an email to set up our initial phone
consultation.
Download these two forms and fax them back to us as
soon as possible.
